Vermont Health Care Assessment Contribution

Recently, the State of Vermont passed a law requiring a quarterly Health Care Assessment to determine if additional monies must be reported with your unemployment tax return. To determine if a Health Care Assessment Contribution is due for this reporting quarter, complete the Vermont Department of Labor (HC-1) Health Care Assessment Worksheet.

If it is determined that you owe an additional quarterly Health Care Contribution amount, fax the completed Assessment Worksheet, including your company name and federal identification number, to the Intuit Tax Department at (800)490-2934 by July 12, 2008. Learn More...

Step 3: Company

  • State Withholding and Unemployment Tax identification/account numbers

  • Payroll items (wages, tips, deductions, etc.)

  • Employee default payroll settings

Step 4: Employees

  • Employee list
  • Payroll and tax information for all employees
  • Employee Direct Deposit information (if applicable)

Step 5: Year-to-Date

  • Year-to-date payroll information
  • Prior tax and liabilities payments information
  • Copies of all current calendar year Federal and state withholding reports and coupons
  • Form 941 for the third quarter of 2000 through the last quarter filed
