To determine if a Health Care Assessment Contribution is due for this reporting quarter, complete the Vermont Department of Labor (HC-1) Health Care Assessment Worksheet. If it is determined that you owe an additional quarterly Health Care Contribution amount, fax the completed Assessment Worksheet, including your company name and federal identification number, to the Intuit Tax Department at (800)490-2934 by July 12, 2008.
